Q62E1 Every year, retirees receive a form for tax preparation purposes that is like the W2s that employed persons receive. Have you received that form (1099-R) yet? If so, the amount would be on that form.

Q62E2 Railroad retirees receive a form for tax preparation purposes that is like the W2s that employed persons receive. Have you received that form (RRB-1099) yet? If so, the amount would be on that form.

Q63c If you already did your taxes, did you have to fill out a Schedule B?

Q74a Ask about two of the better known retirement plans which are the 401k plans offered by some employers in private industry and the Thrift Savings Plan offered by the Federal Government.

SHI6 We are concerned with how much the employer or union paid toward the insurance premiums not medical bills. Ask the question again emphasizing or explaining that we are concerned with health insurance premiums.

SHI6 Tell the respondent you are concerned with the health insurance plan they had the longest. If they worked for both employers for the same amount of time, then they should focus on the health insurance plan for the most recent job.

Q80 Does he eat a complete hot lunch offered at school more than half the week?
